Elder Connect is a community-based care management agency that assists aging and disabled adults locate the resources and services necessary to manage their health conditions and maintain their independence, safety and integrity. Not only do we assist our clients with locating the best providers and services, but we also provide routine monitoring to ensure that these services continue to meet the client's changing needs. We are the one stop shop for senior care services. We help our clients put the pieces together.

We can assist with household chores, respite, errands, transportation, medication reminders and more.
