We are interested in joining your team as a provider, we are a RSA level 3 agency. We provide skilled care: Nursing, Psychical therapy, and occupational therapy. We also provide non-skilled for personal care assistant. listed below are the services we offer to our patients.
-General assessment and nursing care of patient in the comfort of their homes post hospital discharge
-Interdisciplinary approach, Team care, planning, coordinating, and delivering personalized health care, working together with other healthteam towards same goals to provide the best outcomes and patient care -Duties includes but not limited to monitoring of patients ,wound care, G tube care, iv therapy, various knowledge of pumps ,trachea care, ventilators, catheterizations, TPN, disease process and management, medications management ,generating plan of care[GOALS AND INTERVENTION] personalized to each patient conditions ,coordination of care of patient with their primary care physician and caregivers. -Administration of medications -Completion of OASIS paperwork -We offer personal care services such as activities of daily living under Medicaid and private duty. We also provide occupational therapy and physical therapy services. -Coordination of patient health care condition and making/reporting recommendations as needed we are a home health agency were our extensive skills and knowledge would be utilized as well as providing room for professional advancement and personal growth. PT. to evaluate/access and develop PT plan of care, that would be approved by the physician prior implementation.
