It would be helpful to have someone you know you can count on to be there to stand in the place of a family member at appointments or provide care and support in the home. Continued Care Nursing Services provides a service that is unique from the rest.

Our nurses can come to the home, hospital or physician s office to provide care, understanding, clarity and recommendations to assure continued health care and wellness is a priority. We advocate for you.

We provide support and education for those with diabetes, cardiovascular disease, stroke, cancer and those suffering from chemical addiction, relapse and mental health issues. We also provide hourly and live-in companion care.


Initial assessment Includes:
Blood Sugar Monitoring and Blood Pressure Checks
Fall and Safety Risk Assessment
Skin assessment/wound care
Nutritional Assessment
Medication Review

Addiction Counseling and Recovery/Relapse Support* (urine drug testing upon request)

Our nurses and coaches come out weekly to support, encourage, guide and discuss the essential tools needed to lead a sober and productive life. There will be weekly assignments, referrals to recovery groups, and occasional recreational outings with other clients and your mentor. The weekly service will include a 24 hour hotline and unlimited support.
