Iwork with retired professionals & the aging loved ones from those families in the SANDWICH GENERATION who want to AGE IN PLACE and enjoy their golden years regardless of their health challenges. I help my clients to live life on their own terms and to keep their dignity in the place where they are most familiar.....the place they know by heart....their HOME. I provide full medical management, in-home caregiving, chronic illness management, symptom management, post hospital and post rehab care. I specialize in post hospital and post rehab care especially for those with movement disorders, Parkinson's, PSP, stroke & generalized muscle deconditioning who have lost the ability to care for themselves. I serve my clients & patients through 3 departments. #1. IN-HOME CAREGIVING: This is supportive living services in the comfort of your home, condo, apartment, your independent or assisted living suite. When needed, we will take extra care into an assisted living facility. Trained, experienced, insured & bonded caregivers provide meals, grooming assistance, medication reminding, home making, companionship & socialization. #2. POST HOSPITAL CARE, POST REHAB CARE, CHRONIC ILLNESS & SYMPTOM MANAGEMENT. Sometimes after a hospital or rehab stay, older adults lose their ability to walk or care for themselves. The goal of this service is to help older adults to get back on their feet as fast as humanly possible. We offer restorative care & functional fitness along side & in collaboration with the skilled therapies from Medicare. Even when it is time for hospice, we are here to help. #3. PRIMARY CARE & MEDICAL MANAGEMENT: When there are too many hospital trips & too many medications, this service brings primary medical care into your home. WHAT TO EXPECT?
