CHRONIC ILLNESS AND SYMPTOM MANAGEMENT: these services are designed to reduce frequent trip to the hospital and rehab facilities. The care program are designed by nurses. CARE MANAGEMENT AND CARE COORDINATION: these services are provided by geriatric care managers.
