ComForcare's commitment to quality care begins with our compassionate and proactive approach to care. This process begins with identifying clients' and families' unique needs, creating a patient-centered care plan, coordinating with physicians and other providers, and implementing quality assurance throughout the duration of care. We have a rigorous 10-step screening process for our caregivers, nurse oversight of client care, and ongoing training and evaluation at every level.
If you're starting to look forin-home elder care for a loved one, it can be an anxious time. Not knowing what to do next is perhaps the hardest part. Here is a basic overview of what you can expect.
Calling ComForcare
Our highly-trained staff are ready to talk with you about your needs and answer your questions. Once we understand your situation, we'll provide you with elder care options for your loved one, describe our services in detail, and schedule an in-home visit to conduct a thorough needs evaluation. Give us a call.
Home Visit
We give families the opportunity to get to know us better during the in-home visit. One of our staff members will evaluate your loved one's needs, as well as those of your family. We will set goals for care, perform fall prevention and home safety checks, and discuss costs and funding options.
Nurse Evaluation
Once the level of care has been agreed to, one of our experienced nurses will visit to perform a complete assessment of your loved one's physical, mental and social needs. The nurse will use the assessment to develop a personalized care plan that will be followed by the caregiver at every visit.
Matching Caregiver to Care Recipient
We want your home care experience to be successful. A key to that success is a warm and trusting relationship between your loved one and their caregiver. We consider everything we've learned about your loved one to assign someone who best matches your loved one's personality and needs.
Follow-up, Quality Checks and Reevaluations
To ensure that your loved one is receiving the highest quality of care, we touch base with you or your loved one at the end of the first shift, the first week, the second week, the first month and every month thereafter. We also perform in-home supervisory visits of our caregivers and reassessments of your loved one to ensure their needs are being met and to make adjustments to the care plan as needed.…
