Catholic Charities Day Break Cares offers in Home Care and Adult Day Care services to Seniors and their loved ones. This also includes our Elder Care Line providing confidential support, encouragement, and referrals for elder care service needs. Our professional elder care specialists understand the needs of seniors as well as their loved ones and offer a host of services and support when you need it most. Our services range from in Home Care Services and Adult Activities Centers to dementia care and coordination of services to fit your needs. Our team will help you think through options and create solutions that fit your individual and family needs.

As a non-profit organization we serving over 54,000 individuals annually through 80 different direct service programs. Established in 1955, CCSCC's mission is to serve and advocate for families and individuals in need. Through 60 different community sites, CCSCC provides a wide variety of services such as: older adult services, mental health services, immigration legal assistance, refugee resettlement, supportive housing, job skill training and placement, ESL and computer training.

Through the Older Adult Services Division, CCSCC serves over 15,000 seniors annually along a continuum of care, which emphasizes independence, health, and advocacy for seniors. CCSCC develops and implements innovative services adapted to the specific needs of older adults and their families. Programs include: home care, adult day care, senior neighborhood centers, meal programs, long term care ombudsman, behavioral health, senior nutrition programs, etc.
