At Care Core we offer Patient Advocacy, Navigation, and Care Coordination Services. Through these services we assist our clients by providing requested information, logistical support, guidance, and emotional support. We facilitate our clients' ability to make informed choices regarding available options and resources within the Healthcare system, thus removing barriers to their care in the home, hospital, or nursing facility.
We have a specific Case Management process we use that conforms to each individual client and their care plan to bridge any gaps in communication with all members of their Healthcare Team. This was designed with the direct intent in keeping our clients involved in all aspects of their care as their needs evolve.
We also offer Concierge Services to assist our clients with daily living tasks such as, Companionship Services, Home-Helper Services, and Errand Services.
