Combining the expertise of the founders, which include a Pharm.D., Registered Nurse, and Engineer/MBA Graduate, we have come up with a unique business model that integrates Chronic Care Management seamlessly into Traditional Care Giving Services.

OUR CHRONIC CARE MANAGEMENT SERVICES ARE PROVIDED AT NO ADDITIONAL COST TO OUR CLIENTS. OUR RATES ARE EXTREMELY COMPETITIVE AND RANGE FROM $16-20/ HOUR!
Every Be Well Client Receives:
-A Patient Care Plan that is Developed by the RN Supervisor which is Catered to
the Client's Specific Needs
-Care Provided by an Aide who has received Highly Specialized Training from a
Pharm.D. and Based off of the Client's Individual Health Issues and Care
Needs Enabling them to become an Integral Part of the Client's Chronic Care
Management Team.
-Assurance that the Aide Selected for them by the Agency has Passed a
One on One Skills Assessment Test Conducted by the RN Supervisor
-An In-Depth Analysis of the Client's Medications Performed by the Pharm.D.
Every Quarter and Upon Hospital Discharge. The Pharm.D. Assesses if there
are Any Ways to Help the Client Save Money on Prescriptions and to Assess &
Provide Recommendations for Any Compliance Issues. Then She Checks to
See if There are Any Issues with the Medication Regimen such as Drug
Interactions, Duplication of Therapy, Unnecessary Medications, Need for
Additional Medications, or Ways to Optimize the Current Therapy. The
Pharm.D. then Submits a Professional Report which Includes Therapy
Enhancement Recommendations, Health Metric Graphs, and Any Important
Observations to the Patient's Primary Care Physician.
-Pharmacist as an On Demand Resource to Answer Questions about
Medications.
