We are a 100% Minnesota-owned and operated home care company that has been providing home care to Minnesotans for 20 years. Our mission is simple: To provide personalized in-home care that makes it possible for adults from 18 to 100+ to continue living their best possible life, at home.

At Baywood Home Care , the care and assistance that we provide is similar to the care provided in assisted living and nursing homes, except that it is one-to-one care in the comfort of the client's own home.

Each Baywoodclient has a Care Manager who is a Registered Nurse. The Care Manager meets with the family and client to create a personalized care plan for the client. the R.N. Care Manager is involved with the client's care on an ongoing basis, by making frequent visits to the client's home to update the care plan, consult with the client, and supervise our caregiver. Baywood caregivers are well-screened. They are home health aides with Home Health Aide or Nursing Assistant training. They have prior experience providing care to seniors and other adults, and also have passed a rigorous skills test, and background, driving record, and employment reference checks. Baywood provides nursing home-level care at home. Our care and services include housekeeping, meal preparation, medication administration, monitoring of vital signs, bathing and personal care, catheter care, tube feeding, use of mechanical lifts, assistance with mobility, providing companionship, driving clients, etc. We also work very hard to ensure that our clients have consistent caregivers, so our clients can count on the same caregivers coming back week after week. The R.N. Care Manager communicates frequently with the client and their family. The Care Manager's role can include being an advocate for the client's best interests, managing medications, accompanying the client to doctor appointments, arranging therapies, consulting with doctors and families, ordering medical equipment, or arranging Medicare services (e.g. physical therapy). Baywood has a Registered Nurse on-call 24-hours per day, 7- days per week for questions or emergency needs.
