A licensed nurse will come into your home and complete an Admission Assessment. The admission assessment is composed of a health assessment, history assessment, skin assessment, needs assessment, and baseline vital signs. The needs assessment will identify how to properly provide care for you including feeding, bathing, mobility, etc. You the consumer have the right to be involved in the service planning process and you will receive the services with reasonable accommodation of your needs and preferences. Ourcare planning process will identify all aspects of your care along with psychological/psycho-social needs as well. The care plan will be completed and placed in a company binder and all caregivers will be trained on how to properly care for you. We believe that continuity of care is key to promoting great caregiver/client relationships. The administrator will at all times attempt to deliver continuity of care by having you or your loved one cared for by the same caregivers on a daily basis. While that is our goal that cannot always be maintained due to career changes and emergencies. The care plan designed will ensure that any caregiver that enters your home understands how to care for you.
