Our Personal Care Attendant (PCA) care plan/documentation of services form must have instructions and comments about the residents needs for assistance and any special instructions or procedures required. The month to month plan for the use of this PCA services is part of the PCA care plan. A copy of the plan will be provided to the PCAs providing the services. This must be updated as needed when there is a change in need for PCA services. A new PCA care plan is required annually at the time of IEP and service reassessment.
