Provides MD, RN, LVN, home care aide, social worker, clergy and volunteer visits per each individuals needs. Medications ARE delivered to the home. All equipment needed: i.e. hospital bed, bedside commode, wheel chair, walker, cane, oxygen, etc. per individual patient needs. Diapers and bed pads for incontinent patients are supplied. Wound care supplies for patients with wounds are supplied. Patient, family and caregiver education in all areas of comfort care is provided. The services are paid for by Medicare, Medi-Cal and most all private insurances. No out of pocket expense unless there is a share of cost for Medi-Cal. We will work with the patient/family regarding any shore of cost. Mose emergency Medi-Cal patients can get full Medi-Cal benefits under hospice. We can service patients at home, in a nursing home, in an assisted living facility, in a small 6 bed board & care ANYWHERE the patient resides. You may terminate care anytime you desire if you find it's not for you. Signing a DNR is not required and can be signed later on during the service delivery. Diagnosis covered (but not limited to) are: End stage Cancer, Alzheimer's, Heart disease, Liver disease, Parkinson's disease (or other neurological disease), Lung disease, Kidney disease, Stroke and HIV and AIDS. These should be present: weight loss, physical/functional decline, cognitive decline, swallowing difficulty, requires total or or maximum assistance with (3) bathing, feeding, dressing, walking, toileting, and transfers.
