Established in June 2002, our mission is to facilitate the delivery of professional and ancillary health care services in the patient's home setting in order to accomplish the following goals: <br/> <br/>To maintain patients in their homes as long as possible and provide services in the least restrictive setting <br/>To assist patients to return to their optimal level of function <br/>To develop a relationship with referral sources to effectively meet our patient's home care needs <br/>To function as a liason between our patients and available community resources and assist our patients in coping with their needs and problems.
