Services include: In person consultation and evaluation to determine the services that may be necessary to maintain a self-reliant lifestyle; Educate seniors and their family on available resources and guide them through the process; Create a service management plan as needed; Coordinate and monitor delivery of services and supervise and manage when needed; Act as a liaison between Seniors, providers and family members; Provide quality, certified Homemaker Companions.
