Our Mission is to assure that children and families are safe, healthy, and connected to resources available within their communities. We have over 20 years of experience servicing families with intellectual, developmental, disabilities, physical disabilities, and those with severe emotional disturbances. We take pride and handle each family with a sense of priority as we all deserve exceptional and passionate coordinators to service.
We do this by:
-helping participants deal with Social Security
-assistance inmanaging personal finances
-assistance with Medicaid and Food Share benefit applications or renewals
-assistance as a liaison with healthcare providers
-assistance with end-of-life and funeral planning
-assistance with coordinating complex issues
-assistance in the transition from high school to adult services
-assistance in identifying social, recreational, and educational opportunities to
increase community involvement
